Review these requirements carefully prior to contacting us for an Application appointment.  If you are unable to provide all of the requested information and documents, you will not be eligible for hire.
 
Valid drivers license (ID is not acceptable)
Social Security Card
Current vehicle insurance with your name listed as a driver
Have reliable transportation available for your own use when working.
Pass a Motor Vehicle Screening
Pass a Criminal History check (state and national).
Pass a Possible drug test
Be at least eighteen years old
Must be able to read, write English, follow verbal and written instructions and.
document the services provided
Must have a minimum of six to twelve months of satisfactory and verifiable employment
experience in senior care
Work references with:
     Employer Name
     Phone Number
     Supervisor Name
     Position Held
     Dates of Employment (starting and ending months and years)
    Reason for Leaving
Personal references with:
    Name
    Phone Number
    Number of years known
    (Cannot be the same person included as a work reference or related by blood or marriage.)
